Simple Store feature

Would like to customise the checkout in the simplestore for my products in a way that customers can answer my customised questions. So instead of just having categories like colour or size, I can set up my own question for my product. Example: if you are a florist you might want to add a personal note to the florist or "what would you like to say on your card?" Or if im designing a custom image, I can ask them specific question or even upload their own design, logo, etc.
